Chronic Care Management (CCM) is the coordination of care that is initiated by and/or results in a patient or patient-related contact with other health care providers via phone or electronic communication. RPM is a perfect complement to CCM to better engage patients with chronic conditions to achieve their quality of life goals.
Chronic Care Management & Remote Patient Monitoring
Physicians and other Qualified Healthcare Practitioners (QHCPs) can bill for RPM and CCM concurrently as long as the minimum clinical time requirements are met for each CPT code, each billing period.
